1 annual wellness visit, with basic wellness labs included through Fullscript
Up to 4 chronic-care follow-up visits per year
Up to 5 acute or urgent-care visits per year
Women’s primary care and weight management
Mental health medication management (non-controlled)
Patient portal and care coordination for routine needs
After included visits: $75 per extra follow-up, $100 per extra urgent visit. Not included: vaccines, procedures, controlled substances, specialty care, emergency care.
